rTMS of the left DLPFC awakens hedonic response in cocaine addiction: a case report

Addiction is a chronic, relapsing disorder related to reduced self-control and associated with severe negative consequences. Among substance of abuse, cocaine is highly prevalent and cocaine abuse represents a major public health concern.
Repetitive transcranial magnetic stimulation (rTMS), a non-invasive brain stimulation technique, has been used in experimental approaches to a variety of neuropsychiatric disorders and, recently, it has been evaluated in addiction disorders.
We report the case of a single cocaine addicted patient, treated with DLPFC rTMS. We observed cocaine abstinence, improvement in cocaine craving measures and overall symptomatology, and the reversal of dampened hedonic response to natural rewards after treatment. We hypothesize that these results are likely related to improvement of PFC control and consequent modulation of dopaminergic and glutamatergic neurotransmission. Further investigations on
larger samples are necessary, to better explore the anti-craving and relapse-preventive effects of this promising treatment.
